Before Your First Appointment
In order to better treat you, we need to know about your past medical history, medications or supplements (including herbals) that you are taking, any allergies you may have, and your family and social history. To help speed your check-in time, please fill out the following forms and bring them with you:

Checking In
Please bring your most current insurance card for your primary and secondary insurances along with your photo ID. Photo IDs are now required as part of HIPAA regulations. Your privacy is protected and the practice maintains compliance.
Please bring your co-pay. In addition to personal checks, we accept Visa and MasterCard for your convenience. We are required by your insurance company to collect your co-pay at the time of your visit.
Your Appointment
We encourage you to bring a family member or caregiver with you to your appointment. Your doctor will be providing information to you about your condition and an extra set of ears will helpful in asking questions.
After Hours Care
A physician is on call 24 hours a day, 7 days a week. After hours, call our main number (831-475-2220) and leave a message with your name, phone number and your question. We strive to return your call promptly. The phone number in our records may not be the number you're currently calling from, so don't forget to leave the best number to reach you!
Holidays
The office is closed on the following holidays: New Years Day, Memorial Day, Independence Day, Labor Day, Thanksgiving and Friday, and Christmas. After hours care is available during holidays.
